Do you have a positive reference or evaluation from your previous work experiences?

- Current photographs in uniform or business/formal attire on white background
- 1 pc 4R size, whole body photo
- 1 pc 4R size, half body photo
- Government ID’s/Numbers
- SSS
- Pag Ibig
- Philhealth
- Clear Photocopies of the ff:
- Training Certificates
- Transcript of Records and/or Diploma
- Certificate of Employment (from previous jobs)
- Passports (1st Page)
- Seaman’s Book (1st Page)
- All Current Visas
- NSO Birth Certificate
- BT w/COP
- SDSD w/ COP
- COC
- Medical
- Licenses (if applicable)
